Revive Physiotherapy -Terms & Conditions + Privacy Policy

1. Introduction

These Terms & Conditions (“Terms”) govern the provision of physiotherapy and related services by REVIVE PHYSIOTHERAPY SYDNEY – ABN: 69442380161 – (“we”, “our”, “us”) to you (“the client”, “you”). By booking and attending a session, you agree to these Terms.

2. Services Provided

We provide physiotherapy home visits and related services in accordance with professional standards and the guidelines set by the Australian Health Practitioner Regulation Agency (AHPRA) and the Physiotherapy Board of Australia.

3. Appointments & Cancellations

Appointments must be booked in advance.

If you need to cancel or reschedule, at least 4 hours’ notice is required.

Cancellations made with less than 4 hours’ notice will incur a cancellation fee of 50% of the session fee.

A valid credit card is required to secure your appointment.

Please note: No payment will be taken at the time of booking. Your card details are securely stored and will only be charged if you fail to attend your appointment or cancel with insufficient notice

If you fail to attend an appointment without notice (“no-show”), the full session fee may be charged.

We reserve the right to reschedule appointments due to unforeseen circumstances.

4. Fees & Payment

Fees for services are set out in our current fee schedule and are subject to change with notice.

Payment is required on the day of service unless otherwise agreed.

We accept (cash / card / EFTPOS).

For NDIS/Medicare/DVA/Aged Care/Private Health Insurance/WorkCover clients, services will be charged according to the NDIS/Medicare/DVA/Aged Care/Private Health Insurance/WorkCover Price Guide.

5. Health & Safety

You agree to provide accurate and complete health information before and during your treatment.

You acknowledge that physiotherapy involves physical movement and carries potential risk.

We are not liable for injuries, conditions, or complications that arise where you have withheld information, failed to follow advice, or not complied with treatment recommendations.

6. Privacy & Patient Consent

We reserve the right to modify these terms at any time. Continued use of our services implies acceptance of the updated terms.

We are committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. To provide quality health care, we may collect personal information and medical history from you. Your information will be used solely for the purposes for which it was collected, which may include:

  • Administrative and billing purposes, including Medicare compliance.
  • Appointment reminders and recall notices (e.g., via SMS, email, or phone).
  • Disclosure to other healthcare providers involved in your care.
  • Accreditation and quality assurance activities.
  • Legal disclosures as required by law.
  • Research and staff training using de-identified information.
  • Compliance with legislative and regulatory requirements.

Your information will always be treated confidentially and securely. By engaging our services, you consent to the collection, use, and disclosure of your information for the purposes outlined above.

You may request access to your personal records or ask for corrections at any time in line with our privacy obligations.

7. Liability

To the extent permitted by law, our liability is limited to the resupply of services or a refund of the service fee.

Nothing in these Terms excludes rights you may have under the Australian Consumer Law (ACL).

8. Changes to The Terms + Conditions and Privacy Policy

We reserve the right to modify these terms at any time. Continued use of our services implies acceptance of the updated terms.

9. Governing Law

These Terms are governed by the laws of New South Wales, Australia. Any disputes shall be subject to the jurisdiction of the courts of that state.
